How would you file this motion?

My fianc茅s babymom has violated their court order 3 times, 1) she submitted her boyfriends number as the "neutral" 3rd party instead of her brothers number 2) she showed up with her boyfriend to swap their son wherein court order states her brother is to appear not her (%26amp;definitely not her boyfriend) 3) she is not cooperating with the parenting time schedule. My fianc茅 has missed two weekday visits and 1 weekend. What does he need to do as far as filing a motion and also, he wants to ask the judge for custody of his son. If she is breaking the "law" by having her boyfriend with her to "swap" kids - its probably because her brother has to work, and she is supporting her boyfriend so he'll have time for this nonsense. Your fiance just needs to keep a journal of when and where the baby's mother failed to keep her appointments - and submit to the Judge that handled the child's custody arrangements.
